Dear dr. Topol: This videoblog is a great idea and will hopefully add great value to TheHeart.org. There are several links scattered through these web pages, announcing symposia and educational material that address the issue of restoration of sinus rhythm in AF. One of them deals with its impact in patients with chronic heart failure and new-onset AF. However, just a few hours ago, a new study has been presented in AHA 2007, suggesting that restoration of sinus rhythm in stable HF patients may not be necessary, at least, as far as hard endpoints are concerned. I would appreciate to hear your comments on this new study.

Dear Sergio, Not unusual to see mixed data like this, with under-powered studies. As you know, most of the data supports rate control is adequate but the larger, randomized trials to date have not enrolled CHF are insufficient or conflicting data, individualizing the approach to the the patient (and using clinical judgement) seems to be the best approach.

The true value theheart.org and this new initiative lays in the daily comments by leading experts produced as soon as new findings are presented or are published. This indeed helps a lot to translate clinical research into daily clinical practice.
