As continued refinements in catheter-based device technology continue to occur, further increases in the number of interventional procedures will likely result. Despite the demonstrated clinical benefit of reperfusion strategies, substantial challenges exist in identifying the precise combination of therapeutic agents and interventional procedures that will maximize patient outcomes. In addition, intra-procedural complications occur and remain an important source of patient morbidity and mortality. Despite advances in the standard of care for high-risk patients with acute coronary syndrome (ACS) and evidence-based guidelines recommending antiplatelet agents as the cornerstone of therapy, existing therapies still have considerable limitations. Currently, dual therapy with aspirin and a thienopyridine is the standard of care, but new combinations of antiplatelet agents and new drugs with potent antiplatelet effects, such as AZD6140, cangrelor, and prasugrel, have been evaluated in clinical trials, and these agents may ultimately improve outcomes in patients with ACS.
